Denial and Closure of Services 650-25-45-65

- An individual (not currently enrolled in the program) who is ineligible for and denied services under the FCSP may submit a grievance as outlined in this section.
- FCSP services may be discontinued if the caregiver has not accessed services within a consecutive three-month period.
- FCSP services will be discontinued when the caregiver or care recipient, or both, no longer meet the program requirements.
- A caregiver will be notified in writing of the reason for denial or closure, the right to submit a grievance, and the grievance process through the NDFCSP Notice of Service Denial or Closure (SFN 331). The SFN 331 is not required if the closure is due to the death of the caregiver or the care recipient.
- Any individual or agency who applies to be a qualified service provider, is enrolled as a qualified service provider, or is a qualified family member providing caregiver services through the Family Caregiver Support Program is subject to rules outlined in the North Dakota Administrative Code Chapter 75-03-23-08: Termination of qualified service provider status and denial of application to become a qualified service provider.
